Our Process

Step 1

You or someone acting on your behalf should contact your local authority to request a social care assessment. This is to establish your eligibility to receive care under the 2014 Care Act. The local authority will then carry out a financial assessment to work out funding.

Step 2

Complete our electronic referral form below or alternatively download our referral form here. Please complete as much details as possible with all the mandatory spaces completed and send it to admin@genesis-hsc-services.co.uk with ‘referral’ as the subject heading. This is usually carried out by a social worker or a representative within the local authority.

Step 3

Our manager will be in touch with you to arrange a more thorough assessment. Ideally, this would be with other representatives, family and friends so we can get to know you better and establish how we can support you.

Step 4

We will liaise with the local authority about our assessment and wait approval for funding. Once approved, we will agree a visit and move in date to your chosen accommodation and begin an individualised care plan.
